1. Do we offer group health plans as well as personal, family and seniors heath plans? Yes – absolutely. 2. What advantages do we offer you? Insurance is a controlled market service because in the same state the same plan from the same insurer costs the same, no matter what insurance agency you use or don’t use. So, we do not offer you any price advantage or disadvantage on an apples-to-apples comparison of the same insurance plan. Beyond that, the similarities stop and our advantages start to accumulate. They begin with our consultative approach to help you get the most appropriate health plan at the lowest price. Next, you get the benefits of our Fitness Culture programs that will help you better manage your healthcare affairs, your health service quality and cost considerations, your wellness and fitness and longevity issues, and your healthcare system components. You’ll get the information, advice and assistance you need, but can’t find anywhere else, to help you make better decisions in less time and at lower cost. 4. Do all of those Fitness Culture benefits cost you more? A bit, depending on the number of insureds involved. But, the costs are almost negligible in comparison with the cost of health insurance plans and with the cost-benefit values of the Medical Self Care Plan, of the Total Fitness Plan and of the Patient Care Plan. These plans combine to build a wellness mindset that can positively transform a workplace or a lifestyle. 5. How do you know that your personal information will be safe? In over 15 years of working with confidential client information, we have never violated this client trust. Our client relationship is too valuable to jeopardize. Our

assures your information will be handled with the greatest care. It is our normal procedure to ask our clients for permission to notify you from time to time by email of various services and products that we think may be of interest to you. You will have the option to accept or reject any of those offers or inquiries. 6. How do you know you are getting the lowest premiums? Our consultative approach for seeking the best solution for our clients has always focused on optimal value, defined as the plan design and quality of service that is most appropriate for the client at the best price for that model and quality level. We do not simply seek the lowest premium cost. That is not necessarily the best choice. Besides premium, plan design factors that contribute to overall cost and value include deductible, co-pay, co-insurance, out-of-pocket aggregate payments and claims frequency and adminiistration. Our objective is to help our clients achieve the plan design that gives them the opportunity to minimize their overall healthcare and insurance expense while maintaining quality health services. Our Fitness Culture Plans can contribute significantly to achieving those objectives at an overall lower cost because they help minimize claims and maximize wellness and quality of life. 7. Can we help you if you have a pre-existing condition that many private insurers will not cover? Probably. Pre-existing conditions are often not an issue with larger group plans or with insureds who have continuity of coverage. They can be a concern for small group and individual or family plans. With small group and personal plans, an insurer does not have the broader pool to diffuse risk of loss. A quote from an insurance company is an offer to provide coverage for the insured on a basis commensurate with the risk assessment the insurer perceives. In the absence of required coverage that spreads risk, a pre-existing condition may set up a sure loss for the insurer. No business wants to lose money, so they either decline to offer or make a conditional offer. Usually, the best quote an applicant with a pre-existing condition can expect is an offer that is limited or conditional in some manner. An insurer may offer coverage at higher premium rates or coverage that is limited in time or amount as to the specific pre-existing condition, or some combination of all. Because different insurers have different underwriting criteria, one company may cover what another excludes. If no private insurer will offer coverage, an assigned risk plan can be an option in some cases. The only way to know for sure is to ask. 8. An application may be processed in a day or so, unless there are questions that need to be answered or additional information is needed. 9. Can I get a group health insurance quote and apply online? Yes. Go to our

A group plan can require significant information, depending on the number of families involved. It also requires employer risk assessment information for underwriting. Once all of the information is available, processing can often be done in about two weeks. Most risk assessment information can be emailed or faxed to expedite handling. 10. If I get an online quote, am I obligated to buy? No, you are not obligated to buy. A quote is an offer by the insurance company to provide the coverage on the premium cost and terms stipulated. If you accept the quote and pay the initial premium, the coverage can be bound as provided, but you are not obligated to do so. However, continuity of coverage is vitally important, so you must be careful to avoid any gaps in coverage by delaying past a prior policy expiration date. And, you must take care not to cancel prior coverage until replacement coverage is absolutely assured and in place. 11. If you are not satisfied with your healthcare services or a health insurance service or claim response, can we help you? Yes. Through our

If you are a small employer who can’t afford the high cost of an employee health insurance plan . . . how can we help you? For employers who can not afford to pay the total premium for employee health plans, our Fitness Culture Plans can be a terrific complement to a partial health plan payment or an employer contribution to a health reimbursement arrangement or a health savings account for employees. Another low-cost option for employers can be limited benefit plans that we could set up as a
pseudo-group program
in conjunction with our low-cost
Fitness Culture Plans.
13. If you're a middle-aged individual who lost group health insurance, but you are not old enough for Medicare. How can we help you and your family? We have several options for you. If you need interim or short-term coverage for less than a year, we can probably offer a plan that compares favorably with your COBRA option at a much lower cost. If your need is for longer than a year, we can offer more permanent options that will not only be very competitive but can be complemented by our Fitness Culture Plans. 14. If you're between jobs and have COBRA coverage, but it’s costly . . . how can we help you? Our
Short-Term Health Plans
are attractive alternatives to COBRA and can be offered in conjunction with our Fitness Culture Plans that are not available under your COBRA option. 15. If you get a health insurance plan through us, do you have to get the Fitness Culture programs? No, you could simply take the health insurance plan without the Fitness Culture benefits. However, that would be like eating your brussel sprouts because they’re good for you, then passing on the magical, non-fattening special treat chocolate dessert that’s the reward. Why would you want to do that? 16. Can you get the Fitness Culture programs without getting our health insurance plan? No. We offer the Fitness Culture Plans as a low-cost, value-added benefit for our insured clients. The Fitness Culture Plans are priced to cover costs and minimal margins but are not priced as a profit center that would support offering them as an independent service. The only exception that we make is that we offer the Fitness Plans as an extended family service for seniors and elders of insured clients, with rates adjusted for higher frequency use by seniors.
